This registry is a practice-based quality improvement initiative. It is not a clinical trial, does not constitute IRB-regulated research, and does not replace peer-reviewed clinical evidence. All patient data is submitted de-identified. No patient personally identifiable information (PII) is collected or stored at any point.
By participating, practitioners agree to submit only de-identified data in compliance with HIPAA safe harbor standards. Each case is assigned a randomly generated case ID — the only patient identifier ever stored. Practitioners retain full responsibility for compliance with applicable state and federal laws.
